ENROLLMENT PROCEDURES
First Semester; School Year 2009-2010
ENROLLMENT PROCEDURES FOR REGULAR STUDENTS
(OLD STUDENTS UNDER BLOCK SECTION)
Students must proceed to the following enrollment stations:
- SAF Booth – Pay the Student Activity Fee (PhP 150.00) at the SAF Booth/Terminal beside the Social Hall.
- Social Hall (Dean’s Table) – Present the SAF Receipt and secure Enrollment Form with printed subjects, schedule of classes and assessment of fees.
(Note: If not pre-printed, students will write other needed data like name,
course, student number, year and section.)
- Registrar's Office – Present the Enrollment Form with the signature of the Dean for validation and assessment of fees.
- Accounting Office – Present the Assessment Form and pay the said enrollment fees. However, you may officially enroll with only PhP 1,000.00 as Initial Payment.
(Note: Class cards will no longer be issued.)
Important Note: Kindly present the validated Enrollment Form to your
respective subject teachers during the first day of
classes on June 8, 2009.
ENROLLMENT PROCEDURES FOR IRREGULAR OLD STUDENTS
Students must proceed to the following enrollment stations:
- SAF Terminal – Pay the Student Activity Fee (PhP 150.00) at the SAF Booth/Terminal beside the Social Hall.
- Social Hall (Dean’s Table) – Present the SAF Receipt and secure Enrollment Form. Present the accomplished Enrollment Form to the Dean or to the designated representative for academic advising and approval.
- Registrar’s Office – Present the Enrollment Form with the signature of the Dean for validation and assessment of fees.
- Accounting Office – Present the Assessment Form and pay the said enrollment fees. However, you may officially enroll with only PhP 1,000.00 as Initial Payment.
(Note: Class cards will no longer be issued.)

(FRESHMEN AND TRANSFEREES)
Students must proceed to the following enrollment stations:
- Social Hall (Admission Table) – Submit the requirements and secure Recommendation for Enrollment in the chosen College with the Entrance Examination Result.
- SAF Terminal – Pay the Student Activity Fee (PhP 150.00) at the SAF Booth/Terminal beside the Social Hall.
- Social Hall (Dean’s Table) – Present the SAF Receipt and secure Enrollment Form. Present the accomplished Enrollment Form to the Dean or to the designated representative for academic advising and approval.
- Registrar’s Office – Present the Enrollment Form with the signature of the Dean for validation and assessment of fees.
- Accounting Office – Present the Assessment Form and pay the said enrollment fees. However, you may officially enroll with only PhP 1,000.00 as Initial Payment.

- ID Section – Present the Official Receipt from the Accounting Office for the processing of School ID.
